Evolution of technology for proton and ion beam therapy

Abstract: High power lasers have been shown to have the capability of producing high energy particles, however the character of the beams is not nearly as refined as what is available from accelerators. Monoenergetic beams of 250 MeV (and lower energy) protons have been available for over 70 years, and were first applied for medical treatments in laboratory settings in the 1970’s. The first clinical proton facility, at Loma Linda, started treatments in 1990. The growth of the field has been very high, today there are at least six commercial vendors marketing turnkey proton facilities, all offering the most advanced pencil beam scanning delivery. The learning curve for these vendors has not been easy. Adapting the pristine monoenergetic proton beam to clinical use was a much more complex task than anticipated. The first commercial company almost went bankrupt in meeting the stringent requirements established for the first unit they delivered, the second company did fail, and was bought out by a much larger company which did understand the differences between a physics machine and a clinical instrument. The laser community is just embarking on this learning curve. It may not require the full 70 years it has taken accelerator protons to achieve clinical maturity, however the journey must not be underestimated.

  • Is hadron therapy better than existing therapies and for which tumours?
